As with all medical care, there are several forms that we need to have filled out. Those forms can be downloaded here and printed using Adobe's Acrobat Reader so you can fill them out before your first visit, or if you're not able to do that please call our office and we'll be happy to mail a registration packet to you.
The forms we use are:- Registration Form
- A standard registration form for your name, address, employer, emergency contacts and insurance.
- Patient History
- A fairly detailed medical history relating to your reasons for visiting us. We need to know about medicine allergies, medications, your obstetrical history (if any), your gynecological history, etc. Because you may need to look up much of this information it's best if you fill this out well in advance of your initial appointment. We encourage you to keep your own copy of this history for your records as well.
- Privacy Policy
- This describes how we may and may not use your health information and what you may ask us to do or not do with it. We need this form signed as an acknowledgement that this information has been provided to you.
- Financial and Appointment Cancellation
- These forms simply outline a few additional policies for referrals, payment plans, missed or cancelled appointments, and medical records requests.
